At Claudia Criswell Nutrition we adhere to the principles of the Data Protection Act 2018. We do not share personal information with third parties unless we have your permission or are required by law to do so.

The data controller is Claudia Criswell Nutrition of 56 Rodney Street, EH7 4DX, and we are responsible for your personal data (referred to as “we”, “us” or “our” in this privacy notice).

If you are not happy with any aspect of how we collect and use your data, you have the right to complain to the Information Commissioner’s Office (ICO), the UK supervisory authority for data protection issues (www.ico.org.uk). We would be grateful if you would contact us first if you do have a complaint so that we can try to resolve it for you.

It is very important that the information we hold about you is accurate and up to date. How do we collect information? 

You may provide data by filling in forms on our site (or otherwise) or by communicating with us by post, phone, email or otherwise, including when you: order our products or services, subscribe to our service or publications, request resources or marketing be sent to you, enter a competition, prize draw, promotion or survey, or give us feedback, complete a health questionnaire or booking form, attend a live or online talk, event, webinar or workshop. 

What information do we collect?

When registering for our services we collect personal and health related information. This information is supplied by you when entering data on our website or filling in questionnaires. We also collect information during consultations with you. These consultations could be in person, video call or by telephone and includes email correspondence.

Why do we collect your information?

We use your information to help us fully understand your health concerns so that we can provide nutritional services tailored to your needs. The information may be shared with our carefully chosen partner laboratories, other practitioners involved with your care with whom we may need to discuss your test results for purposes of interpretation.

Information may also be shared if:

  • You ask us to share your test results and/or reports with your GP.

  • We think it advisable to refer you to other healthcare professionals. This will be discussed with you.

  • Requested by Care Quality Commission (England) or Care Commission (Scotland) upon inspection. The CQC/Care Commission assess safety and quality of the services offered by the clinics we work from.

How do we store and protect your information?

We are committed to protecting and respecting your privacy.

Do you have access to the information we hold.

In accordance with the Data Protection Act 2018 and Access to Health Records Act, clients may request to see their nutritional records. Requests should be made by email to claudia@claudiacriswell.com.

In compliance with GDPR, patients have the right to:

  • Access all of their information plus any other content that forms part of the patient record, including notes and expect to be able to read them and understand what they mean without expert medical knowledge.

  • Know if their personal information has been forwarded to a third-party (like a fellow healthcare professional, consultant, insurer or school).

  • Have any invalid information we hold about them corrected.

  • Apply to have their personal data deleted.

  • Ask us to refrain from further use (or processing) of their information.

  • Receive their information in an open electronic format.

  • Be notified if critical information about them is inappropriately accessed.
